UNDER THE PLAN, AM I REQUIRED TO PRE-CERTIFY A HOSPITAL STAY?

If you are receiving care from an out-of-network provider, you will need to pre-certify your hospital stay; if you do not precertify, certain penalties may apply. The number to call for pre-certification is listed on the back of your ID card. If you are receiving care from an in-network provider, he or she will make this call for you.
