IS THERE A SEPARATE DEDUCTIBLE FOR EACH COVERED DEPENDENT?

Yes. Each dependent must reach the individual deductible up to a combined maximum deductible based on the coverage level you selected (Employee + 1, Employee + 2 or more). Note that in-network services only count towards the in-network deductible and out-of-network services only count towards the out-of-network deductible. For more detailed information, click here.
