WHEN THE LM HEALTHWORKS PLAN PAYS FOR AN IN-NETWORK OFFICE VISIT, IS IT PAID AT THE FULL RATE OR NEGOTIATED RATE? HOW IS MY HEALTHFUND AFFECTED?

An in-network office visit is paid at the negotiated rate. The negotiated rate amount is the medical expense used for purposes of the annual deductible and is automatically deducted from your HealthFund, if you have a balance.
