Nominate Your Provider


If your doctor is not in the LM HealthWorks Plan network, you can nominate him or her for membership by:

  • Visiting the the provider search tool and clicking on “Nominate a Provider” in the menu on the left side of the home page. Just complete the online form and click “submit” at the bottom.
  • Download this letter and bring it to your doctor. It describes how he or she can apply for network membership online by going to the Join the Aetna Network page on the Aetna.com website.

Before using any of these options, be sure to verify that the doctor is not already a member of the network. You can visit the provider search tool or call your doctor’s office to do this. Keep in mind that the nomination process can take up to 3 to 6 months.
